An orthopedic visit usually starts with imaging — X-ray, MRI, CT — that needs to sit next to the exam note, not in a separate PACS viewer staff have to switch to mid-visit. A surgical case adds a full episode of care: pre-op clearance, the operative note, and a post-op recovery timeline that can run months, often handed off to physical therapy along the way. General EHRs track each piece adequately in isolation but rarely connect them, which is how a referral to PT quietly falls through the cracks or an imaging result gets reviewed without the full surgical history in view.
